The Benelli Lift (concentric, or peri-areolar, donut or doughnut lift, Diagram 2): This technique is considered less invasive and was designed with the incisions being made around the areolae (or areolas). With the Benelli, a doughnut-shaped piece of tissue around the areola border (or includes areolar tissue as well to reduce its size) is removed and the surrounding tissue sutured to the areola. The incisions are sometimes closed with permanent purse string sutures. The Benelli lift results in flatter projection, yet a rounder breast shape postoperatively as opposed to a naturally sloped breast.
The Benelli Lollipop, or simply Lollipop (or Keyhole, Diagram 3): This lift is the same as the above but with the addition of a straight scar from under the areolae to the mammary fold (crease). This is for those who have medium ptosis, too much for the Benelli only and too little for a full anchor incision. Puckering at the edge of the areola is possible.
Full Breast Lift (anchor, Diagram 4): The most commonly used technique for those with severe ptosis is with an anchor-shaped incision that starts at the base of the areola, then vertically to the where the breast crease meets the rib cage and then along the lower portion of the breast at the natural crease (or slightly higher). Nipple re-positioning is sometimes necessary with this technique as the nipple must be partially removed (see above) and left on a pedicle of flesh to retain the blood flow. Clients who present to the hospital for surgery, who are under 20 years of age without prior authority and consent will be denied surgery and PIAC will not be responsible for any costs,loss or damages.
Thankfully, breastfeeding after implants in not an impossible feat. Most women do it and do it with great success. However, the milk production in the breast with implants would be much less as compared to a normal lactating breast. This would mean that exclusive breastfeeding may not be possible as the baby grows. You might have to introduce formula milk along with breastfeeding if the breast milk is insufficient for the baby.
Can You Breastfeed With Implants?
Most likely it depends on the kind of surgery you had, but most approaches are compatible with breastfeeding. Incisions made under the fold of the breast or through the armpit shouldn’t cause any trouble. A “smile” incision around the areola increases your risk of having breastfeeding problems.
That said, there are no guarantees that you will be able to breastfeed, regardless of whether you undergo breast augmentation. Women who have never had any breast surgery may experience difficulty breastfeeding.
If you do opt to have breast surgery, incision and implant placement may make a difference in terms of your ability to breastfeed. For example, in rare cases a periareolar incision (around the edge of the nipple) may interrupt or disturb the milk ducts. Placing implants below the pectoral (chest) muscle is least likely to disturb the milk ducts. Ultrasound doesn’t pick up the minute calcifications of early breast cancer. And it is very operator dependent; results vary tremendously based on who is performing and viewing the image. Ultrasound is an adjunct test used to distinguish between solid and cystic masses or to help assess very dense breasts. The current practice is not to use it as a primary screening tool to detect breast cancer in women without or with breast implants.
Does having Breast Implant increase higher incidences of breast cancer?
No. Many studies have found that breast cancer incidence in women who have had implants is actually lower than that of the general female public. One study from Sweden followed 3,400 women who had received cosmetic breast implants between the years 1965 and 1993. They had an average follow-up of 18 years. The women with implants were found to have a 30 percent reduced risk for breast cancer when compared to women without implants. Another study, this time in the U.S., compared breast cancer incidence rates among 13,000 women who had breast implants with expected rates based on U.S. surveillance. And here too the risk was slightly lower. It’s possible, though many feel not medically probable (or even statistically proven) that breast implants may decrease breast cancer incidence by:
- Compressing breast tissue and interfering with its blood supply.
- Decreasing temperature in the breast.
- Stimulating an immunological response, which helps destroy potential breast cancer cells.
It may be, however, that the apparent decreased incidence of breast cancer in women with breast implants reflects the fact that these women are more likely to be thin (obesity is a significant risk factor for breast cancer). Also, the women with implants tend to be in a higher socioeconomic group with better nutrition, exercise, and cancer screening. They see their physicians, especially the breast specialist more frequently, have more frequent mammograms and better access to medical care than their cohorts who don’t have implants.
When cancer is found in women with implants, is the disease at a later stage?
This has been addressed in another study published in The Journal of the American Medical Association (JAMA), which used the data collected from the National Cancer Institute funded Breast Cancer Surveillance Consortium (BCSC). They compared 141 women with augmentation and nearly 21,000 without who were diagnosed with breast cancer between 1995 and 2002. The tumors were diagnosed at a similar stage and size, but tended to be lower grade (less worrisome) for women with breast augmentation compared to those without. The authors concluded that “although sensitivity of screening mammography is lower with breast augmentation, there’s no evidence that this results in more advanced disease at diagnosis compared with women without augmentation.”
